The Collaborative Action Programme works in partnership with Mental Health Services for
Older People and the Royal District Nursing Service to support older people with multiple and complex needs in their community.
The Collaborative Action Programme provides a range of practical and social and treatment options to enable
older people who have a mental illness and complex needs to have an enhanced quality of life, and the support to thrive in their own homes and communities.
Services are provided within a Mental Health Reovery Framework to enable people to increase their independence and community participation.
Clients
Older people who are over 65 years of age and Aboriginal and Torres Strait Islander people over 45 years who have dementia or mental illness or both and who demonstrate challenging behaviours
People eligible for this service may have difficulty accessing mainstream programs
or are at risk of premature admission to institutionalised
care, and may have limited social connections and/or are
estranged from family members.

Referrals to this service are made through the various Mental Health Services for Older
